In November of 2023, we engaged the PCN Communities on the barriers to keeping frail seniors living in the community. You can view summary reports from the engagement sessions here. One of the key pieces of feedback we heard was more support to understand what resources and services there are in Vancouver for frail seniors, and how to navigate them. The Seniors and Frail Elder Committee and the PCNs are excited to partner again to host an information session on March 13th from 6 – 8pm. In this session, you will receive information about how you can further support your frail senior patients through the Vancouver PCN Interprofessional Team, Vancouver Coastal Health services and Community Partner supports. Presentations will be followed by time to ask questions about these services. We hope you can join us!
