The Vancouver Division of Family Practice Long Term Care Initiative (LTCI) enables physicians who work in long term care and commit to a formal relationship with a care home to receive additional funding if they meet six identified best practice expectations in their care home work. The six best-practices include regular proactive visits; on-site medication reviews; attendance at care conferences; involvement in 24/7 on call; completed documentation in charts; and attendance at quarterly meetings with other care home-attached practitioners and care home leadership.
Remuneration: LTCI care home panels are remunerated through fee-for-service. All physicians who join the LTCI and take on a panel between 5 to 35 beds also qualify for the following incentive payments:

Practitioners working in LTCI care homes will work with an interdisciplinary care team: physicians, nurse practitioners, nurses, allied health, and the care home leadership team
Crofton Manor is a 90-bed long term care home currently being supported by four Long Term Care Initiative (LTCI) practitioners. One practitioner is leaving and therefore an additional practitioner is required to assist with the resident caseload.
