Attachment Reporting
As per the Clinical Services Collaboration Agreement, when a Vancouver Coastal Health (VCH) Registered Nurse (RN) is placed in a clinic, that clinic (not a specific physician), is expected to attach 500 net new patients by the end of the 3-year contract.
All family physicians and nurse practitioners are required to use the same zero-fee attachment code to identify new patients who are attached to their practice as described in the PCN Service Plan Approval.
Below, please find the attachment code for all Vancouver PCNs, as well as information on how the Patient Attachment Initiative can support you meet the attachment criteria.

Encounter Coding Reporting
PCN-funded RNs are required by the Ministry of Health to Encounter Report through the Medical Service Plan Teleplan system for services provided.
