The PMH Connectivity and Digitization Program is back!!
The Vancouver Division of Family Practice is pleased to announce the return of the Patient Medical Home (PMH) Connectivity and Digitization Program. This program is designed to support Vancouver-based, family practice clinics in enhancing their cybersecurity, improving their connectivity to provincial systems and optimizing their technology-related workflows.
This year, you will be able to contact the PMH Connectivity & Digitization Program’s Tech Concierge team through two avenues:
Request support over email or phone
Attend the Tech Concierge’s virtual office hours

Provincial System Connectivity
A number of Provincial systems are available to PMHs to help improve your clinic’s delivery of primary care. The Division will provide one-on-one guidance in determining your clinic’s connectivity needs and helping you navigate connectivity to provincial systems such as:
- CareConnect and PharmaNet for patient data
- Pathways for physician and patient resources
- OceanMD for digital referrals and orders (only available for clinics using OSCAR Pro, Med Access, and Accuro at this stage)
- Health Data Coalition for quality improvement in primary care
Clinic Technologies & Cybersecurity
Every day, Vancouver primary care clinics are required to interact with myriad technology and technology adjacent tools and workflows. Sometimes things just don’t work as expected. At the same time, new technologies are available to help improve your clinic workflows and give you back precious time.
The Tech Concierge can help you find out more about clinic workflow solutions, and help connect you to the right resources and experts on the problems or questions you have.
Meet Andy Wynden, the Vancouver Division Tech Concierge
I have been a Tech Concierge with the Vancouver Division since 2024, and find it is a real privilege to work with an organization whose sole focus is serving its members.
I love helping computers and humans understand each other better, and get really excited about how we can better design our systems to serve its users.
I am still amazed at how complex the landscape of clinic technology is, and am excited to help you navigate this complexity.
Outside of work, I live as an off-grid guest on unceded MÁLEXEŁ (W̱SÁNEĆ) territory where I enjoy gardening, land stewardship, building things, and getting creative.
Last Year's Program
During our 2024/25 program (June 2024 – March 2025), our Tech Concierges provided personalized support to 70+ Patient Medical Homes (PMHs) in Vancouver answering hundreds of questions, connecting decision-makers to provincial partners and supporting improvements in technology workflows and digital security.
