Participants will gain a clearer understanding of the intake process, the types of referrals that are accepted or declined, and how IPT clinicians collaborate to support patients through a team-based model of care. This session will feature perspectives from the PCN IPT Registered Dietitians, and the Clinical Pharmacists, and will include practical examples to illustrate the IPT approach to supporting patients living with menopause. This session will be structured as follows:
- Overview of menopause management within the IPT context
- Overview of IPT roles involved in menopause-related referrals, including Registered Dietitians and Clinical Pharmacists
- Intake process and referral criteria (accepted vs. declined referrals)
- How IPT clinicians support patients with diabetes through a team-based care model
- Case examples demonstrating the IPT approach
- Facilitated discussion and Q&A
