Seeking a panel takeover of approximately 625 patients starting mid-February 2026. The current panel includes a mix of family medicine patients across all age groups.

The nurse practitioner would be responsible for comprehensive primary care, including chronic disease management, preventive care, and episodic visits.

Strong administrative and clinical team support is in place to ensure continuity of care and a smooth handover.

This Vancouver clinic is looking to have a Nurse Practitioner join their clinical team. As part of a multi-disciplinary approach to full-service family medicine, an interested NP would work collaboratively in the team environment and start their own patient panel as per Ministry requirements.

This will be a full-time or part-time salaried position. This clinic is excited to add to their team a Nurse Practitioner interested in working together to provide primary care for the surrounding community of patients. This initiative is part of Primary Care Networks (PCNs) evolving in Vancouver. NPs interested in this opportunity should be willing to work collaboratively and participate in PCN development in the community.
