Community-based family physicians can now use a new regional form to refer patients for Home and Community Care services from Vancouver Coastal Health (VCH).
Based on feedback from physicians and divisions of family practices, the refreshed VCH form streamlines 14 previous versions as a one-page, fillable PDF that is easily accessible. The new form can be sent to any VCH Home Health intake team in the region, expediting intake and getting patients the care they need sooner.
Upon receiving the form, the intake team will perform the standardized, evidence-based intake screening interRAI-Contact Assessment with the individual in care or someone in their support network to help identify needs, risks, priority level and appropriate care referrals.
Physicians can access the VCH Regional Home and Community Care form on Pathways BC.
If you are a primary care provider without Pathways access, please reach out to the Pathways team by selecting the “Request access” link on the login page, and a team member will be in touch with you.
Committed to helping people who have ongoing or chronic health issues to live safely at home, VCH Home and Community Care services and personalized care are delivered by interdisciplinary teams and include home support, overnight respite, palliative and long-term care, adult day programs and medical equipment provisions. Support for families and caregivers is also available.
