Resources to Support the Delivery of Palliative Care

The Vancouver Division recognizes the importance of providing support for our community. Below are Palliative Care tips, resources and programs we hope you’ll find useful.

We extend our appreciation to Dr. Kelly Little for her valuable contributions in curating the Palliative Care Tips content. Her expertise has greatly supported primary care providers in delivering high-quality palliative care. As of October 2025, Dr. Andrew Jeong will assume the role of Palliative Care content contributor. We welcome Dr. Jeong and look forward to his insights and contributions in advancing palliative care in the community.

Palliative Care Champion

Dr. Andrew Jeong was raised in Burnaby, BC and is a graduate of the St. Paul’s CMG family medicine program. He previously served as a resident board member for the Vancouver Division of Family Practice. Currently, he is undergoing enhanced skills palliative care training at UBC. His interests are in improving the intersection between primary care and palliative care by bridging research, policy, and advocacy. He hopes to support family physicians in delivering high-quality, pragmatic palliative care.

Past Palliative Care Contributor

Dr. Kelly Little’s writing on palliative care in Vancouver focused on enhancing clinical support, education, and collaboration within the medical community. Through her posts, she emphasized three key themes: Practical Tools for Palliative Care, Spotlighting Speciality Leaders, and Building Palliative Care Community. Her overarching goal is to make palliative care more accessible, collaborative, and well-supported within Vancouver’s medical community.

Steps for diagnosing and staging frailty – By Dr. Kelly Little

I diagnose and stage frailty using the Rockwood Clinical Frailty Scale. It matters: frailty predisposes patients to negative outcomes and death. I use a Frailty Roadmap for Families to have hard but important conversations. I share my plan with all members in the circle of care when able.

Expected Death in the Home Forms
EDITH Forms Resources – By Dr. Andrew Jeong

Family physicians have an attending physician role to fill EDITH forms ahead of time, which allows for family members to waive pronouncement of death by a healthcare professional and allows for seamless transfer of the deceased patient to the funeral home.
