PCN Engagement Forum: Empowering Care Across Life’s Moments

Join members of the PCN 4, 5 & 6  communities at the first in-person PCN Event in over 3 years. Network with your community, hear the latest PCN updates and provide input to support and empower care, focusing on two patient populations that need more support from the primary care system: maternity patients and frail seniors. This input will begin to help us understand how to broaden support for these patients through the PCNs.

Dinner will be provided.

Who will be in attendance:

• Primary Care Providers (family physicians and nurse practitioners) primarily providing care in PCNs 4, 5 & 6
• PCN Registered Nurses working in PCNs 4, 5 and 6
• Patient Medical Home Leads (clinic owners/leads) in PCNs 4, 5 & 6
• Members of the PCN Patient Partner Advisory Committee
• Executive Directors/Leads of Community Led Healthcare Centres in PCNs 4, 5 and 6
• Members of the VDoFP Seniors & Frail Elder and Maternity Committees

We’ll also have representatives who support the PCNs from Vancouver Coastal Health and the Vancouver Division of Family Practice joining at the event.

Spaces are limited! We will have a waitlist system in place and are also planning more PCN events in the early spring and summer of 2024.
