Team Based Care Education

The Vancouver Division of Family Practice has partnered with UBC Continuing Professional Development (UBC CPD) to develop an education series for teams working together in Vancouver Primary Care Networks (PCNs). A foundation of PCNs, team-based care (TBC) allows teams to work more collaboratively and effectively together to support the needs of their patients. This leads to better experiences for patients and providers alike.

The Vancouver PCN TBC Education Series was adapted from the Practice Support Program’s TBC training. It was built with the support and guidance from a Physician Lead, as well as collaboration with the Vancouver Coastal health Allied Health Practice Leads and staff, The Practice Support Program TBC Team, Family Physicians, Nurse Practitioners and Registered Nurses. We are very grateful for their support.

This education focuses on interprofessional collaboration and aims to address communication barriers, role ambiguity, dynamics between corporate and clinical care issues and more. There are both in-person and online options available for team-based care training.

In-Person Learning

This training consists of 4 x 2 hour sessions – each focusing on a different theme of team-based care:

  • Session 1: Recognizing Your Team
  • Session 2: Understanding Scope and roles
  • Session 3: Optimizing Communication
  • Session 4: Practice Integration & Workflow

Sessions are designed for your clinic team to participate together. They are co-facilitated by a Physician Facilitator and a Community Network Manager, and can be held online via ZOOM and/or in person. Your clinic will receive a summary report as well as connection to a PSP TBC coach both during and after the sessions.

Who should take this training?

This training aims to support clinic teams to optimize working together along with the integration of PCN resources. Consider this training if you currently refer to the PCN Interprofessional Team, are integrating PCN Registered Nurses, working with Nurse Practitioners, or want to optimize how all team members in your clinic work together to support your patients.

Online Education

The TBC online course will teach you the basics of team-based care, from defining your team and understanding roles, to communication skills and integrating workflows to enhancing peer support and team development. The course is approximately 1 hour in length and can be taken as a primer for the 4-part team based care training or as a stand-alone learning opportunity.

Who should take this training?

People working in healthcare teams that are interested in individually learning or reviewing the foundational basics of team-based care.
