Mental Health & Substance Use PCN Engagement Forum: Connect, Engage, Thrive
Come and hear about updates on Mental Health & Substance Use projects as well as be engaged through different discussion topics and resources: suicide supports, alcohol use disorder and motivational interviewing, preparing your patient for substance use support and provider wellness – taking care of yourself through difficult conversations with your patients. As always, there will be an opportunity to network with other folks working in your PCNs and Vancouver at this in-person event.
Who will be in attendance:
- Primary Care Providers (family physicians and nurse practitioners) primarily providing care in PCNs 4, 5 & 6
- Patient Medical Home Leads (clinic owners/leads) in PCNs 4,5 & 6
- PCN Patient Partner Advisory Committee members
- Executive Directors/Leads of Community Led Healthcare Centres in PCNs 4,5 & 6
- Members of the VDoFP Mental Health & Addictions Committee
We’ll also have representatives who support the PCNs from Vancouver Coastal Health, Vancouver Divisions of Family Practice and Community Partner Organizations joining at the event.
