This upcoming event will be hosted by the Vancouver Division of Family Practice's Seniors & Frail Elder Committee. Following the success of our two previous Partners in Care events, we are eager to extend this learning series with a collaborative case studies-focused session. The session will highlight care pathway development, supporting physicians in caring for their frail seniors living in the community. The resources from our previous sessions are available for your review: Partners In Care: Navigating Resources for Frail Seniors, Partners In Care: Preventing & Responding to Abuse, Neglect and Self-Neglect.
