PCN Registered Nurse (RN) Program Working Group

Introduction

The Primary Care Network (PCN) Registered Nurse (RN) Program, launched by the Vancouver Division of Family Practice in partnership with Vancouver Coastal Health (VCH), integrates RNs into family practices across Vancouver. These RNs play a vital role in team-based care, working alongside providers and allied health professionals to deliver timely, person-centred, continuous, and culturally safe care. By enhancing collaboration, expanding clinic capacity, and supporting coordinated care, PCN RNs are essential to stronger primary care. The Vancouver Division, a non-profit funded by the Government of BC and Doctors of BC, continues to partner with VCH to onboard RNs and engage clinics in this program.

To achieve the RN program goals, the Division and VCH are committed to meaningful stakeholder engagement to enable informed decision making and positive coproduction.

In developing a PCN RN working group, we hope to gain diverse perspectives from RNs, providers, and clinics. These insights will help us to better align strategy and objectives to stakeholder priorities, foster a more inclusive approach, and deliver stronger outcomes that have been codesigned with the voices and reflections of those involved in the program.

What Will Your Role Involve? 

  • Bring insights, ideas and feedback to the PCN RN Working Group to help shape and improve the program.  
  • Share your perspectives and experience on PCN RN related topics and issues. 
  • Attend regular meetings and connect offline to help review provider, clinic and client facing documents. 
  • Share success stories and advise on engagement strategies. 
  • Suggest opportunities for program improvement and help develop strategies for the program to serve our local communities. 
  • Advocate for the role of the PCN RN in clinics and support provider engagement and understanding.
  • Foster and pioneer a team-based care approach, collaborating with a multi-disciplinary team of healthcare professionals, other advisory groups and partners.

We’re Seeking Providers and RNs Who: 

  • Provide longitudinal primary care in the city of Vancouver. 
  • Are currently based at a Vancouver Primary Care Clinic with a co-located RN. 
  • Are driven to support Primary Health Care improvement, through integrated team-based and patient-centred care. 
  • Are comfortable collaborating with a diverse group of health care professionals bringing together a mix of perspectives and experiences.  
  • Works well with others in a professional, solution-focused, and consensus-driven environment to support shared decision making and meaningful improvement. 

Members of the PCN RN Working Group will be expected to bring a spirit of collaboration, respect and understanding.

The deadline to apply is midnight on the 6th October. Shortlisted applicants will be invited to interviews in October.  

To enable meaningful coproduction, participants will have an active role in advising program delivery and decision making. Successful candidates will be invited to a session in November to co-design the Terms of Reference for the working group, with the opportunity to develop the scope, suggest activities and influence the group schedule. The Working Group session series will likely commence in January 2026. 

Participants will be expected to contribute approximately 2-3 hours each month to attend regular meetings with the group and connect offline via email. 

Compensation:  

Providers will be compensated at the standard sessional rate.

Background Information

What is the PCN RN Program?

The Primary Care Network (PCN) Registered Nurse (RN) Program started as a partnership between the Vancouver Division of Family Practice and VCH to integrate RNs into existing family practices across Vancouver. The co-located PCN RN resource supports Team-Based-Care (TBC) through building part of an inter-disciplinary team of healthcare professionals with diverse skillsets and expertise who can provide coordinated and comprehensive care. The PCN RNs are an integral part of the success of the co-located teams they serve in ensuring that clients, families and caregivers have access to timely, person-centred, continuous and culturally safe quality health care.  

The Vancouver Division in collaboration with VCH continue to onboard PCN RNs into primary care clinics, through engaging clinics and providers on the role, scope and benefits of PCN RNs, determining clinic interest and eligibility, and supporting onboarding workflow and processes. The program aims to support greater collaboration between providers and RNs, enabling TBC and expanding primary care capacity. 

What is the Vancouver Division of Family Practice ?

The Vancouver Division of Family Practice is a member-driven organization supporting primary care, working to create an engaged community and a collaborative healthcare system in Vancouver. The Division is a non-profit society funded by the Government of BC and Doctors of BC, and we work in partnership with the BC Ministry of Health, Vancouver Coastal Health (VCH), Providence Health Care, and other community organizations. 

What is a PCN? 

A Primary Care Network (PCN) is a clinical network of providers in a geographic area where patients receive expanded, culturally safe, comprehensive care and improved access to team-based primary care. PCN networks include Registered Nurses (RN), Primary Care Providers, Allied Health Supports, as well as other services in the community.
