PMH and PCN Programs Impact Survey

Introduction

This survey is intended to gather information on the progress and value of the support provided by the Vancouver Division of Family Practice and our partners to further two primary care system transformation initiatives:  Primary Care Networks (PCNs) and Patient Medical Homes (PMHs).  If you are a primary care provider (Family Physician or Nurse Practitioner) with a full or part time practice, or a PCN Registered Nurse, working at a Vancouver-based primary care clinic, we would like to hear from you.

The feedback you provide in this survey will support future planning of PMH and PCN supports, and will be included where applicable as part of the overall evaluation of PCNs.  Your responses to the survey are anonymous; any excerpts used in reporting will ensure that the identity of the respondent cannot be determined.

Please note that this survey is separate from the Interprofessional Team (IPT) Provider Experience Survey or PCN Registered Nurse Experience Survey that you may have filled out recently. The intention of this survey is to speak to the collective impact of various PMH and PCN supports and initiatives.

This survey should take no longer than 15 minutes to fill out, and your responses are anonymous. We are very grateful for your input and participation.

PMH & PCN Programs Impact Survey

Family Physicians and Nurse Practitioners will be compensated at a sessional rate for 15 minutes. Once you have completed the survey, you will be redirected to a separate unconnected survey to collect information for payment processing. Payment will be made via EFT so if you do not have any payment history with us, please complete this EFT form and submit to pcn@vancouverdivision.com.
