The Vancouver Divisions of Family Practice and Vancouver Coastal Health (VCH) are working together to implement the 6 geographically-based PCNs in Vancouver (see map below).
The Primary Care Networks (PCNs) are about communities of dedicated doctors, nurse practitioners, and allied health providers coming together to provide patients with greater access to a range of primary care supports.

A Primary Care Network (PCN) is a clinical network of providers in a geographic area where patients receive expanded, culturally safe, comprehensive care and improved access to primary care services. Each PCN has 4 core service areas:
There are 6 geographically-based PCNs in Vancouver as well as the First Nations and Aboriginal PCN which supports indigenous primary care needs through clinics located across the Vancouver Coastal Health service delivery area.
Community Network Managers (CNMs) are engaging the community to build Vancouver’s PCNs. They do this in many ways including regular check-ins with family practices to connect them with local community PCN programs and services, connect with community PCN partners, participate in PCN Steering Committees and maintain a strong understanding of the community PCN needs.
