Our Role in a Collaborative Healthcare System
The Vancouver Division exists to support family physicians and their practices with resources, tools and initiatives. Through our team of dedicated program staff, our mission is to improve the primary care system in Vancouver for the benefit of patients and members alike.
This website serves as a central repository of all programming from the Division, so please visit often, save your favourite news and resources to your account, and engage with your colleagues. Reach out to us anytime you need support for your practice, we’re here to help.
Vancouver's Primary Care Networks
Vancouver is divided into 6 regional Primary Care Networks (PCNs), each working to create a new system for introducing more integrated, team-based and patient centred care in the community. The First Nations and Aboriginal PCN supports indigenous primary care needs through clinics located in Vancouver. Get in touch with your PCN’s Community Network Manager for more info.
