Expected Death in the Home (EDITH) Forms

Family physicians have an attending physician role to fill EDITH forms ahead of time, which allows for family members to waive pronouncement of death by a healthcare professional and allows for seamless transfer of the deceased patient to the funeral home. These forms are in effect for 3 months from the date of signing and also require a community DNR form to be in effect. More information can be found here and forms can also be accessed on Pathways.
