PCN 3 Northeast Food Security Resources

This food resource connects patients, care providers, and community partners with a range of supports aimed at addressing food insecurity. Designed to assist both healthcare professionals and the community, this guide highlights local services including Indigenous food programs, income-based diet supplements, and community food programs. It also features food resource directories, free or low-cost meal services, and links to other relevant health and social supports. By bringing together these tools in one place, this resource supports collaborative care and empowers patients to access nutritious food and holistic support in their own communities.

This resource was created as part of a project led by the PCN 3 Steering Committee, made up of Family Doctors, a Nurse Practitioner and Patient Partner. The committee is supported by the Vancouver Division of Family Practice, an organization that supports family practices and primary care improvement in Vancouver.
