The Vancouver Division and the Vancouver School Board (VSB) brought together 20 family physicians (FPs) and 20 VSB staff to identify challenges in sharing the right information between each other. Key lesson from the discussion was that often information is shared that inadvertently creates barriers to provide timely and appropriate feedback and perspective.

The result the creation of following fillable forms that lay out the key information that either primary care providers or school staff specifically find most helpful. The first is for FPs to use and the second is for VSB staff.

• From Primary Care Providers to School Staff – This 2-page fillable form is a communication tool for primary care providers to share student information with the student’s school staff. • From School Staff to Primary Care Providers – If you are working with a school teacher, counsellor, or any school staff, you can share this form with them for any young patient you have. VSB staff can use this form to share relevant information to a student’s family physician that they have concerns or questions about the student’s behaviour. Download the fillable PDF form here.
