What is a PCN?
The Vancouver Divisions of Family Practice and Vancouver Coastal Health (VCH) are working together to implement the 6 geographically-based PCNs in Vancouver (see map below).
The First Nations and Aboriginal PCN is a unique and exciting collaboration of the First Nations and Aboriginal primary care clinics in greater Vancouver and the Sunshine Coast in the traditional territories of the Musqueam, Tsleil-Waututh, Squamish Nations and Tla’amin Nation, working to support Indigenous care needs in the communities they support.
Primary Care Networks (PCNs) in Vancouver
A Primary Care Network (PCN) is a clinical network of providers in a geographic area where patients receive expanded, culturally safe, comprehensive care and improved access to primary care. Requiring a strong foundation of Patient Medical Homes (PMHs), PCNs are designed to wrap services around patients and create a new system for introducing more integrated, team-based care into primary care in the community.

Community Network Managers
Community Network Managers (CNMs) are engaging the community to build Vancouver’s PCNs. They do this in many ways including regular clinic visits and outreach.
