We know that optimizing team-based care can take time and effort, particularly when integrating new Primary Care Network (PCN) resources. The Vancouver Division of Family Practice has worked with UBC Continuing Professional Development to develop a Team-Based Care Education Series and Online Module built to support Vancouver PCNs. The education focuses on foundational elements of team-based care as outlined in the CIHI Interprofessional Framework and supports clinics to identify team values, understand team member scope and roles, optimize team-communication and integrate these elements into clinic practice. In-person sessions are co-facilitated by Physician Champions and the PCN’s Community Network Managers. Ongoing support is provided by the Practice Support Program’s Team-Based Care Coaches.
