The Community Network Managers (CNMs) will be out and about in July and August, visiting clinics in each of the Primary Care Networks (PCNs). This outreach is an opportunity for CNMs to meet Primary Care Providers and Clinic Staff in the PCN in person, share resources and information, promote upcoming events and connect.
