Introduction

PCN Engagement Forums: Empowering Care Across Life’s Moments

Vancouver Primary Care Network (PCN) Engagement forums were held in November of 2023 and provided an opportunity for members of the PCNs to network, hear the latest PCN updates and be engaged on the care of two patient populations that need more support from the primary care system: maternity patients and frail seniors. This input will begin to help us understand how to broaden support for these patients through the PCNs.
