
Join us at this session where we will be taking a closer look at the PCN Approach to Health Bodies Part 2!
In January of 2024, provider experience surveys were sent out to Primary Care Providers (Family Physicians and Nurse Practitioners) as well as IPT Allied Health Professionals working together in the Vancouver PCNs. The surveys are sent out to help assess the functioning and impact of the IPT Program, and also includes feedback from over 300 IPT patients. The last time we sent out a broader experience survey to providers on the Interprofessional Team Program was in 2022. You can login below to view this year’s summary report.
Since 2022, overall understanding of the program by Primary Care Providers has improved which has helped to built confidence and trust in the program. Overall communications has also improved with almost all providers reporting they are receiving notifications about their patients from the IPT. Areas for improvement include sharing more information on referrals, communicating timelines, wait times, and a deeper understanding of referral criteria for social workers and occupational therapists.
Patient experience with the IPT Program has remained high, with almost all patients reporting they are treated with dignity and respect, are communicated with effectively and engaged on their health care goals.
We are grateful to have regular input from our providers and clinicians working together in the PCNs. We are reviewing all feedback and recommendations as we continue to work on improving the program for patients and providers.
