The Primary Care Network (PCN) Registered Nurse (RN) Program, launched by the Vancouver Division of Family Practice in partnership with Vancouver Coastal Health (VCH), integrates PCN RNs into family practices across Vancouver. These PCN RNs play a vital role in team-based care, working alongside providers and allied health professionals to deliver timely, person-centred, continuous, and culturally safe care. By enhancing collaboration, expanding clinic capacity, and supporting coordinated care, PCN RNs are essential to stronger primary care.

The Primary Care Network Registered Nurse (RN) supports Patient Medical Homes to enable:
- Increased capacity for primary care providers to focus on patient care
- Increased attachment of 500 patients over one year
- Increased support for patient education, outreach and chronic disease management
- Improved patient care through team-based supports
Roles & Responsibilities of the PCN RN
The Primary Care Network Registered Nurse (PCN RN) works as part of an interprofessional team to provide culturally safe, person-centered care.
The role includes assessment, health promotion, chronic disease management, care coordination, and follow-up to support patients across primary care and community settings.

CDM Workflow for Diabetes
A RN supported a Provider in their co-located clinic in Vancouver to recall patients with Diabetes for follow up and develop a sustainable workflow for continuous Chronic Disease Management (CDM). The RN was able to recall and reduce the number of patients with Diabetes who are overdue for HbA1c, BP, lipid levels, eGFR or uACR. The RN was also able to review and update the patients’ vaccination status.
Immunization Workflow
A Provider and RN co-located in Vancouver worked collaboratively on a Quality Improvement (QI) project, supported by PSP, to identify patients 56 years and over, who are overdue for immunizations and recall them for follow-up with the PCN RN. The new workflow increased RN utilization, enabled more proactive care, and reduced the number of vulnerable patients ahead of flu season.
