Patient Medical Homes (PMH)
What is a PMH?
The Patient Medical Homes (PMH) model describes how a medical practice can be optimally designed to meet the needs of the patients, the providers, and the community. Implementing the optimal design in a practice setting provides access to quality primary care. How the attributes are achieved is likely to vary in interpretation throughout the province reflecting the diversity of communities and practices.
The good news is that the vast majority of practices in BC are PMHs of some form. We believe that by supporting practices and providers to explore and develop the PMH attributes we will improve access to quality primary care, and by extension, enhance patient and provider experience of the system.
The attributes of PMHs fall into three broad areas:
- Service Attributes (practice/clinician level)
- Relational Enablers of Care (networks of practices and team-based care)
- Structural Enablers of Care

Clinic Business Solutions (CBS) Team
The Vancouver Division created the Clinic Business Solutions (CBS) team to develop and foster the PMH model in community clinics. The CBS team works in support of Community Network Managers (CNMs) and other Vancouver Division teams to offer support through Clinic Business Tools, Legal & Contract Supports, and the Medical Office Administrator (MOA) Program.

MOA Program
The Vancouver Division previously facilitated Medical Office Assistant (MOA) recruitment through an MOA repository program. This program is no longer operational.
Our MOA Program direction now focuses on retention and facilitating a space where physicians can conduct their own recruitment via more effective solutions.

Current CBS Initiatives
The Clinic Business Solutions (CBS) team is actively engaged in a number of short-term engagement projects centred around topics such as:
