Interprofessional Teams (IPT) Resources
This page provides updates and information for Primary Care Providers and clinics accessing Interprofessional Team (IPT) allied health resources in Vancouver Primary Care Networks (PCNs).
The Primary Care Network (PCN) Interprofessional Teams (IPTs) are Vancouver Coastal Health (VCH) Allied Health Professionals and Community Primary Care Providers working together to support a patient’s medical, wellness and social needs. The Vancouver Division of Family Practice and VCH have partnered to implement IPTs as part of the PCN in your region.

Clinic Eligibility
To get access to the Interprofessional Team Services, your clinic must first be onboarded to the program.
If you would like to be onboarded, or are unsure if your clinic has access, then ask your clinic manager or your PCN’s Community Network Manager.
The eligibility of your clinic is based on a variety of factors, including needing an EMR with attached patients. To learn more about getting access and if your clinic might be eligible, please contact your Community Network Manager and they will assist you.
Referring to the IPT Services
Even if your clinic has access to IPT, you must be individually onboarded to get access to the services. This includes an online or in-person meeting with one of the Community Network Managers. Once you are onboarded you can use the referral guides below.

Virtual and In-person Appointments
Appointments are primarily virtual, but there are options for in-person appointments, should this better suit your patients. In person-appointments take place out of two locations in Vancouver.

IPT Engagement Series
This monthly online series focuses on connecting team members working together in PCNs. The goals of the series are to learn, connect and share questions and experiences to help IPT Clinicians and primary care providers referring to the IPT to optimize how they work together to support their patients.
New! IPT Group Visits
IPT Group Visits have started this Spring, to expand patient access! Clinics can continue referring as usual. If you have questions or feedback about IPT group visits, please contact your Community Network Manager.
