PCN Interprofessional Team (IPT) Engagement Series

Introduction

The Vancouver Primary Care Networks brings you an online series to connect team members working together in PCNs. The goals of the series are to learn, connect and share questions and experiences to help Interprofessional Team (IPT) Clinicians and Primary Care providers referring to the IPT to optimize how they work together to support their patients. These events are open to family physicians, nurse practitioners and registered nurses currently referring to the IPT. 

 

Thank You for Joining the IPT Engagement Series!

Thank you to everyone who participated in Series 4 of the Interprofessional Team (IPT) Engagement Series. Whether you joined us for every session, attended a few, or participated for the first time, we appreciate your engagement and contributions throughout the series.

Over the past year, Allied Health Professionals (AHPs) have shared valuable knowledge and insights across a wide range of topics, helping to support collaboration, learning, and patient care within our primary care networks.

 

Looking Ahead: Series 5 Launches This Fall

The IPT Engagement Series will take a break over the summer and return in September with Series 5.

Based on feedback from participants, we have identified several highly requested topics for future sessions. Some of these themes, such as mental health, are broad in scope, and we welcome additional input on specific areas you would like to explore in more depth.

Participant feedback plays an important role in shaping the IPT Engagement Series. All suggestions are reviewed and considered as we plan future programming to ensure sessions remain relevant and responsive to the needs of providers.

Extended Information

In each session, members of the IPT (social workers, occupational therapists, clinical counsellors, clinical pharmacists, and registered dieticians) use different approaches to share information, hold discussions, ask questions, to dive into ways of supporting team based approaches to care.  This is a chance for providers to engage directly with our allied health professionals to understand each disciplinary approach and help foster a team-based approach to patient care. Sessions will be hosted by the  Community Network Managers on Zoom.

This series is planned and supported by an interdepartmental working group which includes IPT Clinicians, the Clinical Operations Supervisor, member of the PCN Engagement Team and a Community Network Manager.
