A Patient Medical Home (PMH) is a family practice clinic that provides primary care and operates at an ideal level to provide longitudinal patient care. Primary Care Networks (PCNs) are built on a foundation of local PMHs, many of them offering team-based care and integrating new PCN resources, such as the Interprofessional Team, Nurse Practitioners and Registered Nurses.

PMH Supports
There are many resources and services available through the PCNs and the Division to support PMHs in Vancouver.
Community Network Managers
Community Network Managers are your first point of contact if you are looking for information or support. Their key role is to support the development of PCNs in Vancouver. They do this by:
- Facilitating the access and optimization of PCN resources such as the Interprofessional Team and Registered Nurses
- Helping you to stay connected with colleagues and activated through PCN events
- Supporting and collaborating on projects that will improve practice and patient care in your community
Clinic Business Solutions
The Clinic Business Solutions (CBS) team provides support to empower clinic leaders to make informed business and financial decisions for their clinics. Their portfolio includes: providing business tools and legal templates to enhance business decision making, conducting engagement case studies to identify gaps and trends in primary care, and working with physician leaders to develop PMH specific solutions for Vancouver’s PCNs. Recruitment & Retention Team
The Division’s Recruitment and Retention Team can support PMHs to recruit Family Physicians and Nurse Practitioners including both locum and permanent roles through their recruitment platform, Work in Vancouver.
Patient Attachment Initiative
The Division’s Patient Attachment Initiative (PAI) is a referral-based program that finds Family Physicians or Nurse Practitioners for unattached residents in Vancouver. They can work with you to help you build your panel and support your patient attachment needs via two streams: 1) the Referral Partners they support and 2) the province’s Health Connect Registry (HCR). Special PMH Support Projects
PMH Connectivity & Digitization Program
This program is designed to support Vancouver-based, family practice clinics in enhancing their IT security, improving their connectivity to provincial systems and optimizing their technology-related workflows.
Clinic Grants Program
The Vancouver Division of Family Practice offers annual clinic grants to support Patient Medical Homes improve clinic operations and build networks.
Flu Vaccine Delivery Program
The Vancouver Division works with MTS Logistics to provide safe, timely and free transportation of influenza vaccine immunizations to eligible clinics, including start up doses.
