Vancouver Division of Family Practice and Vancouver Coastal Health (VCH) have partnered to implement IPTs.
The Vancouver Primary Care Networks bring you an online series to connect team members working together in PCNs. The goals of the series are to learn, connect and share questions and experiences to help Interprofessional Team (IPT) Clinicians and Primary Care providers referring to the IPT to optimize how they work together to support their patients. These events are open to family physicians, nurse practitioners and registered nurses currently referring to the IPT.

In each session, members of the IPT (social workers, occupational therapists, clinical counsellors, clinical pharmacists, and registered dieticians) will review a case study and discuss how they would use a team based approach to care to treat a patient experiencing a specific issue. Participants will then break out into smaller rooms with the allied health professionals to discuss and answer questions that participants may have. This is a chance for providers to engage directly with our allied health professionals to understand each disciplinary approach and help foster a team-based approach to patient care. Sessions will be hosted by the Community Network Managers on Zoom.
This series is planned and supported by an interdepartmental working group which includes IPT Clinicians, the Clinical Operations Supervisor, member of the PCN Engagement Team and a Community Network Manager.
