PCN 6 Featured Service: South Vancouver Community Health Centre
South Vancouver Community Health Centre (CHC) provides a range of health care services in a single location, including access to public and community health nurses, mental health and addiction counsellors, dental clinics for children, speech therapists, nutritionists, youth drop-in health clinics and more. They see people of all ages who live in PCN 6.
Home Health services available include dietary support (primary care providers can refer by calling central intake at 604-263-7377) as well as general support for homebound patients. The CHC also includes a Prenatal & Early Years Program that provides nursing services to support birthing, parents, babies and families. School Health Programs provide immunization and health information to school-aged children, staff and parents. Youth Clinics (up to age 24) provide drop-in, confidential health consultation for all youth – no appointment necessary. Speech & Language Services are available for children up to age 19.
