Vancouver Primary Care Networks
Team Based Care Education
The Vancouver Division of Family Practice has partnered with UBC Continuing Professional Development (UBC CPD) to develop an education series for teams working together in Vancouver Primary Care Networks (PCNs). A foundation of PCNs, team-based care (TBC) allows teams to work more collaboratively and effectively together to support the needs of their patients. This leads to better experiences for patients and providers alike.
The Vancouver PCN TBC Education Series was adapted from the Practice Support Program’s TBC training. It was built with the support and guidance from a Physician Lead, as well as collaboration with the Vancouver Coastal health Allied Health Practice Leads and staff, The Practice Support Program TBC Team, Family Physicians, Nurse Practitioners and Registered Nurses. We are very grateful for their support.
This education focuses on interprofessional collaboration and aims to address communication barriers, role ambiguity, dynamics between corporate and clinical care issues and more. There are both in-person and online options available for team-based care training.

This training consists of 4 x 2 hour sessions – each focusing on a different theme of team-based care:
- Session 1: Recognizing Your Team
- Session 2: Understanding Scope and roles

Facilitate Team Based Care Education
We are currently recruiting Physician Facilitators to support the Team-Based Care Education in-person learning sessions. Physician Facilitators co-facilitate sessions along with a Community Network Manager, and share their own experiences and wisdom around building a culture of team-based care within their clinics and the PCNs.
Think about applying for this opportunity if you are:
- Working in a Vancouver-based primary care clinic
- Providing total team leadership in your clinic, or for a health care team
- Referring to the PCN Interprofessional Team
- Working in a clinic with PCN resources embedded (Nurse Practitioners, Registered Nurses and/or co-located Clinical Pharmacist), or have experience working in interdisciplinary teams
- Interested in PCNs and primary health care improvement
- Collaborative and solution-oriented
Training will be provided (includes shadowing sessions). Each education series will be up to 12 hours of time (session and prep time). We are hoping each facilitator can support 2 series a year.
Sessions can be held in-person or online, with clinics indicating their preference. Physician Facilitators will be compensated at the physician sessional rate for training and facilitating.
