Patient Medical Homes
A Patient Medical Home (PMH) is a family practice clinic that provides primary care and operates at an ideal level to provide longitudinal patient care. Primary Care Networks (PCNs) are built on a foundation of local PMHs, many of them offering team-based care and integrating new PCN resources, such as the Interprofessional Team, Nurse Practitioners and Registered Nurses, to better support their patients.
