In May of 2024, the Primary Care Networks (PCNs) sent out an impact survey to providers working in Patient Medical Homes (PMH’s) in Vancouver. This included Family Physicians, Nurse Practitioners and PCN Registered Nurses (RNs). The survey and summary report speaks to the cumulative impact of the PMH and PCN work on providers. Program specific evaluations conducted over the years offer insights into the value and impact of individual programs and initiatives, but this survey attempted to capture the overall progress in achieving PMH and PCN attributes and the value of the full range of supports available to Vancouver-based providers.
We want to thank all the providers that provided their input to help inform our ongoing planning and quality improvement work. You can login to view the summary report below. If you need support logging in or accessing the summary report, please contact your PCN’s Community Network Manager.
