PCN Summer Outreach

Introduction

The Community Network Managers (CNMs) will be out and about in July and August, visiting clinics in each of the Primary Care Networks (PCNs). This outreach is an opportunity for CNMs to meet Primary Care Providers and Clinic Staff in the PCN in person, share resources and information, promote upcoming events and connect. Do you want to find out more about accessing PCN Resources like the PCN Interprofessional Team or the PCN Registered Nurses? Would you like to talk to your CNM about potentially co-locating a PCN Pharmacist? Do you want to know about the many other Division supports that are available to you? Connect with your CNM to book a time for them to visit you this summer!
