PCN Interprofessional Team (IPT) Online Engagement Series
Get to know eachother better in the PCNs
The Vancouver Primary Care Networks brings you an online series to connect team members working together in PCNs. The goals of the series are to learn, connect and share questions and experiences to help Interprofessional Team (IPT) Clinicians and Primary Care providers referring to the IPT to optimize how they work together to support their patients. To view a summary report from the first 12 sessions click here.
These events are open to family physicians, nurse practitioners and registered nurses currently referring to the IPT.
In each session, IPT Clinicians use different approaches to share information, hold discussions, ask questions, to dive into ways of supporting team based approaches to care. This is a chance for providers to engage directly with PCN allied health professionals to help foster a team-based approach to patient care.
This series is supported by an inter-departmental working group which includes IPT Clinicians and staff supporting PCN development. If you’d like to learn more about the IPT or these events, please contact your Community Network Manager.

Session Summary Reports
Review summary reports from previous sessions.

“Great to see examples of how complex issues can be managed by different team members, excellent reminder to refer these patients rather than trying to do it all myself in a 15 minute medical visit.”
