Due to popular demand, we have added an additional date for our Partners in Care – Improving seniors’ care: interactive case discussions we are excited to extend an invitation to you. This session will be hosted by the Seniors & Frail Elder Committee of the Vancouver Division of Family Practice. This event will build on our two previous Partners in Care events, we are eager to extend this learning series with a collaborative case studies-focused session. The session will highlight care pathway development, supporting physicians in caring for their frail seniors living in the community. The resources from our previous sessions are available for your review; Partners In Care: Navigating Resources for Frail Seniors, Partners In Care: Preventing & Responding to Abuse, Neglect and Self-Neglect.
We hope you can join us on March 20th from 6 – 8pm. Space is limited so register soon to secure your spot. A waitlist system will be activated should we reach capacity.
This event is open to Family Doctors and Nurse Practitioners working in the Vancouver PCNs.
