A PCN Approach to Healthy Bodies

This online series connects team members working together in Primary Care Networks (PCNs). Join clinicians from the PCN Interprofessional Team (IPT) for the next session in this engagement series, diving deeper into the PCN approach to healthy eating, weight management and more.

This event is open to physicians, nurse practitioners and registered nurses currently referring to the IPT.

If you have questions, or would like more information about this event, please contact your Community Network Manager.
